What you’ll need

Documents typically required to apply for OPWDD

Gathering the right documents up front helps avoid delays. OPWDD generally asks for:

Psychological evaluation (less than 3 years old) — including an IQ test with all summary scores and a standardized adaptive behavior assessment

Diagnosis documentation — proof of a qualifying developmental disability (intellectual disability, autism, cerebral palsy, epilepsy, neurological impairment, Down syndrome, Prader-Willi, familial dysautonomia)

For autism — a comprehensive evaluation describing the specific symptoms and behaviors (e.g., ADOS, ADI-R, or CARS-2)

General medical report (less than 12 months old) — such as an annual physical

Social / developmental history (less than 1 year old) — showing the disability began before age 22

For school-age children — the IEP and any psychoeducational reports it references

Mental health records — if applicable
